Appointment day
Here we provide some useful information for the first visit, preparation for open mri, ct scan, digital x-ray, digital mammography, ultrasound screening. On the day of your appointment you will need the following: Photo Identification Insurance card Prescription from your doctor’s office with diagnosis Some insurance companies also require pre-authorization or a referral for your visit. Please check with your insurance provider to see if it is necessary.

Payment methods
Broadway Imaging Center accepts most major insurances. You may call our Billing Department at 818-548-0022 for more information about which insurances our facility works with. For information regarding your covered services and benefits, please contact your insurance carrier. Please note, any co-pay or deductible that your insurance plan requires is due at the time the service is rendered. We accept the following credit cards: VISA, MasterCard, AmEx, Discovery. We also accept debit cards and cash. Still have questions? Feel free to call us at (818)548-0022. Our business hours are Monday – Friday, 9:00 a.m. to 6:00 p.m. and Saturday 8:00 a.m. – 2:00 p,m. We are closed on Sundays.
